StreetReach (Project ERASE and CARE)

StreetReach is a case management app for the Holyoke community. It is used by police officers and recovery coaches to assist clients in the Holyoke community who need help with opioid or mental health related issues. The app enables users to register and monitoring interactions with clients and notifies users when they need to touch base with clients.
